THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Your Rights

• Get an electronic or paper copy of your medical record. We generally will provide a copy or summary within 30 days of your request and may charge a reasonable, cost-based fee as permitted by law.

• Ask us to correct health information you believe is incorrect or incomplete. We may deny the request, but we will explain why in writing within 60 days.

• Request confidential communications, such as contacting you at a specific phone number or address. We will accommodate reasonable requests.

• Ask us to limit what we use or share. We are not always required to agree. If you pay in full out of pocket for a service and ask us not to share that information with your health plan for payment or operations, we will agree unless law requires disclosure.

• Get a list of certain disclosures made during the six years before your request. One accounting per year is free; a reasonable fee may apply to additional requests within 12 months.

• Get a paper copy of this notice at any time, even if you agreed to receive it electronically.

• Choose someone with legal authority to act for you. We will verify that authority before acting.

• Complain if you believe your privacy rights were violated. We will not retaliate against you.

Your Choices

For certain information, you may tell us your preferences, including sharing information with family, close friends, or others involved in your care; sharing information in a disaster-relief situation; and contacting you about fundraising.

If you cannot tell us your preference, we may share information when we believe it is in your best interest or to reduce a serious and imminent threat.

We do not sell your information or use it for marketing without written authorization when authorization is required.

How We May Use and Share Information

• Treatment: To provide, coordinate, or manage your care and communicate with other treating professionals.

• Payment: To bill, collect payment, verify benefits when applicable, and manage accounts.

• Health-care operations: To run the practice, improve care, train staff, conduct quality and compliance activities, and contact you about services.

• Public health and safety: To report suspected abuse or neglect, prevent or reduce a serious threat, report adverse events, and meet public-health duties.

• As required by law: For health oversight, legal proceedings, law enforcement, workers’ compensation, organ donation, medical examiner/coroner duties, national security, and other lawful purposes, subject to applicable safeguards.

Psychotherapy Notes and Specially Protected Records

Most uses or disclosures of psychotherapy notes, when maintained, require written authorization except as permitted by law.

Certain records may receive additional protection under federal or state law. Causeway does not currently operate as a federally assisted substance-use-disorder treatment program subject to 42 CFR Part 2, but records received from another source may remain subject to special protections.

Our Responsibilities

• We are required by law to maintain the privacy and security of protected health information.

• We will notify affected individuals promptly if a breach may have compromised the privacy or security of their information.

• We must follow the duties and privacy practices described in the notice currently in effect.

• We will not use or share information other than as described here unless you authorize it in writing. You may revoke authorization in writing, except to the extent we already relied on it.

Changes to This Notice

We may change this notice and make the revised notice effective for information we already have and information we receive in the future. The current notice will be available from the practice and on our website.
